Job Description
Currently recruiting a Medical Records Technician to provide administrative support services at Joint Base San Antonio (JBSA) - Fort Sam Houston in San Antonio, Texas. Hours of operation are from 0600-1800 not to exceed 40 hours per week or 80 hours in a two-week period. Duty hours include a thirty minute or one-hour (non-compensable) lunch break.

DUTIES OF THE MEDICAL RECORDS TECHNICIAN:
  • Responding to inquiries from patients, their authorized representative, and other third parties, such as law firms, insurance companies,
    government agencies and researchers, by verifying release of information authorization requirements for each request are met IAW AR 40-
    66, HIPAA, and other applicable guidance.
  • Releasing health information IAW AR 40-66, HIPAA, and other applicable guidance, abstracting information from the medical record,
    obtaining supplemental reports, writing factual summaries related to dates and occasion of service, and preparing for provider signature a
    variety of health administrative forms as required by patients having received medical services.
  • Receiving and screening all incoming correspondence requesting medical information, using knowledge and ability to prepare
    correspondence. Communicate orally and in writing.
  • Performing periodic records review, IAW AR 40-66 and The Joint Commission Handbook, of work performed to ensure regulatory
    compliance.
  • Requesting medical records from other military and civilian facilities by written or fax request;coordinating with facilities outside of
    BAMC by telephone for retrieval of records required for urgent care. Screening and logging all incoming medical records and
    documentation from facilities outside of BAMC and forwards to appropriate treating physician, clinic or records file room for inclusion in
    patient medical record.
  • Receiving and processing official/legal requests;i.E., subpoenas, requests for investigative purposes, court summons, risk management
    reviews, etc., and acting accordingly. Prepare certified copies of medical records. Coordinating with the Staff Judge Advocate on all legal
    inquiries prior to releasing information IAW AR 40-66 and HIPAA guidance.
  • Performing medical records management duties;i.E., filing and pulling records, copying records, scanning, converting records to electronic
    media.
  • Preparing cover letters and billing vouchers using typewriter and/or computer. Process medical records involved for medical board and
    requests for VA claims and assisting with processing medical profiles.
  • Processing discharged patient records for inpatient and ambulatory patient visit type records. Retrieve records on discharged patients from
    hospital wards. Print the electronic records and merging documents and electronic records in the prescribed order IAW AR 40-66 and local
    policy guidance.
  • Printing report from MHS GENESIS on hospital patients discharged, picking up records from hospital wards on these discharged patients,
    performing a 100% record accountability of patients discharged ensuring all required records are received. Collaborate with unit clerks, and
    Head Nurses when necessary, to request missing records and/or reports. Identifying and reporting missing medical records not found within
    two days of a patient s discharge from the hospital to the Department Chief.
  • Performing medical record processing duties to include quality and quantitative review on medical records of discharged patients:
    examining and reviewing medical records content for completeness in accordance with regulated documentary guidelines ensuring all
    necessary reports are contained within records and signed by providers IAW AR 40-66. Work with physician and nursing staff to secure
    missing reports or signatures.
  • Assisting in tracking incomplete medical records, according to established procedures, by preparing and publishing a monthly provider
    report from MHS GENESIS of pending records, sending providers this list, and clearing off the report and MHS GENESIS completed
    records as they are received. Monitoring, accounts for, and following up on incomplete records, reporting problem issues to Department
    Chief.
  • Driving and operating a government vehicle for the purpose of transporting medical records between the hospital and Patient
    Administration Division activities.
  • Assisting in providing information, according to established medical record management guidance, to paraprofessionals, physician and
    nursing staff regarding medical record procedures and policies.
QUALIFICATIONS OF THE MEDICAL RECORDS TECHNICIAN:
  • Possessing one year of practical experience or completion of formal training as a Medical Records Technician.
  • Operating a variety of computer software packages (Microsoft Office, Word, Excel, and PowerPoint) to include military specific computer
    programs (CHCS, AHLTA, MCSC MHS ACCESS TO TIPS ON LINE, MHS Genesis, DMHRSI, DTS, and ESSENTRIS).
  • Demonstrating knowledge of a broad range of medical records references, regulatory requirements and standardized medical records
    procedures, methods, and requirements and using such knowledge to determine what medical records are needed to process requests.
  • Applying a practical knowledge of medical terminology and basic knowledge of anatomy and physiology. Understand the contents of
    medical records, medical references, procedures and policies, as well as the medical legal requirements (Federal and State) of medical
    records.
  • High School Diploma or GED
